Season’s Greetings from Bridge to Home

In 2016, the Bridge to Home team has supported over 70 clients. Bridge to Home is an intensive case management program supporting individuals with limited mobility who are also experiencing chronic or episodic homelessness. The team of caseworkers work alongside clients to make them both formal and informal linkages and supports within their community.
