Bridge to Home by Accessible Housing

The Bridge to Home program is a program that focuses on recovery in housing.
Over the course of the year, 90 individuals were served in the program.

We work with individuals that have limited mobility and are chronically homeless while following these principles:

  • Immediate access to suitable housing
  • Choice and self-determination
  • Recovery orientation
  • Individualized and client-driven supports
  • Social and Community integration


We move people who experience homelessness into independent and permanent housing as quickly as possible, and then provide them with additional services and supports as needed. This includes rental subsidies and case management, with the focus of meaningful community engagement and supportive relationships.

Bridge to Home is a 12-24-month, critical time intervention, case management program that supports people with reduced mobility who are also experiencing chronic or episodic homelessness

Bridge to Home Case Managers provide focused, time-limited, case management to support program participants recover from homelessness, reconnect with their community, and build enduring ties and natural support systems.

Case Managers work alongside program participants to make both formal and informal connections and supports within community.

The Bridge to Home program uses a recovery in housing approach that allows our team to collaborate with market and non-market rental opportunities to find the best housing fit for people in our program.

The Bridge to Home program provides limited financial assistance to program participants in the form of rental subsidy.
